I would like to take this opportunity to introduce myself, as well as, give some background information
on my past experience in terms or my professional career. For the past 3 years I have taught in Scott
County Indiana as an Emotional Disturbance Teacher for grades 6th – 12th grade. As an E.D. Teacher for
Austin Middle/Austin High School I learned many important career lessons. I also obtained an
abundance of education myself from my students.
I have a Special Education Degree from Indiana University Southeast, New Albany, IN. My area of
distinction is in the realm of Mild Intervention. I have a license through the State of Indiana as an
educator in Mild Intervention, which makes me fully qualified to teach over 15 areas ranging from Mild
Cognitive Disabilities, Moderate Cognitive Disabilities, Severe Cognitive Disabilities, Profound Cognitive
Disabilities, Emotional Disturbances, Visually Impaired, Hearing Impaired, Communication Disorders,
Learning Disabilities, etc…
I have also spent a large majority of my time not only working with the disability areas, but as an
academic mentor to my students as well. I taught and managed 7 different grade levels all at the same
time, while modifying the curriculum to meet all the student’s needs. This was not an easy task
considering most of my students were on diploma track to graduate. Meaning the academics was just
as important as the disability areas in which I am licensed. I am Highly Qualified to teach most all of the
4 core subject areas as well.
In the evenings I spent my time teaching students who were part of a manifestation hearing and were
placed on Homebound Services as a part of their education. This enabled me to really get the
opportunity to help these students get back on the right track and complete their education successfully.
I am currently looking to teach in a school or working with troubled youths in the Southern Indiana area.

Responsible for students’ instructional implementation that was part of inclusion courses; to ensure
each student was placed in their least restricted environment.
Devised a curriculum setting that was impressive in terms of academic and behavioral standards.
Created a 4 Tier Level‐System was used to monitor individual student progress in terms of social and
behavioral interactions both verbally and physically.
Accommodated all academic content areas including; U.S. History, World Geography, English/Lang. Arts
(6th‐12th), Health Ed., Algebra I, Pre‐Algebra, Algebra II, Science, Biology, Chemistry, Literature,

You must report to the test center by the Report Time(s) shown for each test session above. If you arrive
after the report time, you may not be admitted to the test session. If you have scheduled two one‐hour
tests in the same session, you will be told on test day which test to take first.
What to Bring to the Test Center

* A print‐out of this Admission Ticket
* Photo‐bearing identification with name, photograph, and signature. Your name as given at the time
you register for the test must match the name on the ID you present at the test center. Acceptable
identification includes a passport, driver's license, state ID, national ID, or military ID with photo and
signature (photo credit cards or check cashing cards are not acceptable).
* Several sharpened No. 2 pencils with erasers and a blue or black pen for the essay or constructed
response tests. Mechanical pencils cannot be used. No pencils will be supplied at the test center.
* Layered clothing, to allow you to adjust for fluctuations in room temperature.
* Individuals taking the Art Making Test must have four color photographs or still reproductions of their
own artwork in two different media. Each image must be of a different work and must be clear and
show the complete artwork.
Modifying Your Registration Information
‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐‐
* Changing Your Contact Information: You can change your contact information online or by submitting
an E‐Ticket Correction Form, found at www.ets.org/praxis, to ETS by the day of the test. After test day,
any address changes must be made online or by phone within 15 business days after the test to ensure
receipt of scores.
* Changing Your Test, Test Center, or Date: You can add or change a test or change your test center or
date for a fee of $45. To change your registration, call ETS or mail the Test, Test Center, and Test Date
Change Request Form, found at www.ets.org/praxis, along with your fee. Test or test center change
requests must be received by the emergency registration deadline. Test date change requests must be
received no later than three full days before your original test date. You can only change your test date
to a future test date within the current testing year.
* Changing Your Score Recipient(s): You may change or add any free score recipients up to the date of
the test. Download the E‐Ticket Correction Form, found at www.ets.org/praxis, and send it to ETS (must
be received at ETS by the date of the test) or submit it to the test center supervisor on the day of the
test. After the test date, you cannot change your free score recipients. Paid score recipients and
additional score reports cannot be changed at any time.
* Canceling Your Registration: To cancel your registration and request a refund, mail the Refund Request
Form found at www.ets.org/praxis. Requests must be received no later than three full days prior to your
test date. Refunds are given for test prices and surcharges (if applicable) only. Registration and/or
service fees are nonrefundable. Refunds will be in U.S. dollars. Payments made by credit card will be
credited back to that account.
* Visit www.ets.org/praxis for additional PRAXIS(TM) policies or to download THE PRAXIS SERIES
INFORMATION BULLETIN and change request forms.
